Interprofessional Ambulatory Care (IpAC) Program – Joondalup

The IpAC Program provides services that offer an extension to the GP referred Chronic Disease Management (CDM) plan or through a self-referral process. High quality, interprofessional and evidence-based lifestyle programs are offered to individuals and groups in the community. The aim of these programs is to offer education and support to assist you to self manage your health concerns and optimise your health and wellbeing.

Students from various health disciplines at ECU work with qualified healthcare professionals to deliver your care program. Your participation in this program contributes to the education of the future healthcare workforce. The quality of the care is continuously monitored through the presence of clinical supervisors at all times.
