Provision of Basic Home Services that enable Aged and Disabled and their carers to remain in the community. Targeting Frail aged, dementia, aboriginal, socially disadvantaged, mental health, carers and complex needs. Services are coordinated and managed from office base through a network of field staff. Providing Domestic Assistance, Personal Care, Social Support, Respite, Podiatry, Centre Based Activity, Counselling Support Information and Advocacy, Transport and Home Maintenance. Person centered approach delivered with a high degree of flexibility working to the Community Common Care Standards.
