DESCRIPTION:Injury Matters is proud to be hosting the pre-conference workshop Joining forces: Strengthening the collaborative approach to community-based falls prevention at the upcoming Australia and New Zealand Falls Prevention Society & World Falls Congress 2023 Joint Conference in Perth. \nEstablishing cohesive falls prevention pathways for older adults living in the community is integral to achieving positive patient outcomes for falls. The diverse risk factors for falls call for a multidisciplinary approach and the implementation of collaborative strategies within healthcare to ensure a coordinated and effective approach to falls prevention care in the community. This workshop will enable participants to connect with a range of international health professionals working in falls prevention\, learn from the perspectives of other health disciplines\, and identify strategies for strengthening the collaborative approach to falls prevention in the community setting. \nCommunity-based falls pathways \nWorkshop participants will be taken on the journey of community-based falls prevention pathways through older adult client case studies. Participants will explore the challenges and opportunities unique to differing health disciplines\, strategies for improving communication and collaboration\, and opportunities for integrated care in falls prevention pathways. Participants will also receive a unique opportunity to hear from an older adult community member\, who will share their perspective and experience of navigating the health system for falls prevention. \nPractical application of multidisciplinary case conferencing \nThis workshop will introduce the principles of multidisciplinary case conferencing as a collaborative approach to falls assessment and intervention in the community. Participants will work within interprofessional groups to apply multidisciplinary case conferencing strategies among older adult client case studies. \nConsolidated learning \nParticipants will leave the workshop with their own action plan for implementing collaborative falls prevention care in their respective healthcare settings. SUMMARY:Occupational Therapy home safety interventions for falls prevention
DESCRIPTION:Over half of all falls-related hospitalisations in Western Australia occur within homes\, where older adults spend much of their time. Falls prevention in and around the home is crucial to prevent falls-related injury and many falls can be prevented by making small changes to an older adults’ home environment. An Occupational Therapist (OT) can assist older adults to stay safe and independent by: \n\nAssessing an older adult’s activities of daily living and ability to carry out everyday tasks around the home\nAssessing for fall hazards and applying hazard-reduction strategies and environmental safety adaptations\n\nThis workshop\, for early career OT or OTs that want to work in the aged care sector\, will explore comprehensive risk assessments for falls prevention interventions for older adult clients\, including falls hazard identification and reduction\, and how to use a home modification assessment. SUMMARY:Falls and cardiac health: The link and the risks
DESCRIPTION:Older adults with cardiovascular disease are at an increased risk of falling\, leading to injury. However\, the link and the common risk factors between cardiovascular health and falls is not well understood in clinical practice. Injury Matters is hosting an online webinar for health professionals who work with older adults to better understand the importance and common risk factors to look out for with heart health and falls\, as well as the screening tools used to help identify and mitigate the risk of falling in older adult clients with heart conditions. DESCRIPTION:Balance is the ability to maintain a stable and controlled body position when moving. Balance plays an important role in daily activities such as walking\, bending over to put shoes on\, driving a car\, or grocery shopping. Balance requires our body’s system to work together to control our movements. DESCRIPTION:Falls are a major public health concern – they are common in people aged 65 and older\, and are the leading cause of injury in this age group. Around one-third of generally healthy people aged 65 and older will have at least one fall each year. DESCRIPTION:As people age\, it is common for their visual function to decrease\, which is associated with an increased risk of falling. Older adults with vision loss are twice as likely to fall than older adults without vision loss. Vision plays a key role in the balance and control required to carry out daily activities with older adults relying more heavily on visual information to maintain their balance. Good vision also allows older adults to identify hazards or obstacles in their environment and navigate safely around them. \nInjury Matters\, through the Stay On Your Feet® program\, and in partnership with Vision Australia WA\, are hosting a 2-hour interactive training workshop for health professionals on vision and falls prevention in older adults. SUMMARY:Medication and Falls In Older Adults Workshop
DESCRIPTION:In Australia\, it is estimated that 30% of people aged over 65 years living in the community sustain a fall at least once a year\, whilst in an aged-care facility\, 50% of residents experience one. \nFalls can result from a multitude of factors\, such as the use of medicines. Some medicines can produce different side effects such as drowsiness\, confusion\, blurred vision\, dizziness\, and postural hypotension\, which can all contribute and increase the risk of a falls incident. There are many negative effects that a fall can have on an individual (both acute and chronic)\, including pain and injury as well as psychological impacts such as anxiety\, a decrease in confidence and the development of a fear of falling\, all of which can lead to a loss of independence. \nDue to the magnitude of the issue\, it is important for pharmacists to be able to recognise and identify a patient who may be at higher risk for falls and feel confident in addressing this issue. \nInjury Matters\, through the Stay On Your Feet® program\, will deliver a 2-hour training workshop on ‘Medication and falls in older adults’ with presenters Dr Andrew Stafford and Dr Tin Fei Sim. The workshop will provide participants with the knowledge and confidence to identify and assess for falls risk in older adults and recommend the appropriate interventions. \nThis workshop aims to: \n\nIncreased knowledge of common medicines that can increase the risk of falls.\nIncreased knowledge in deprescribing for falls risk and what the current evidence suggests.\nIncreased skill in identifying patients who may have an increased risk of fall and discussing ways to prevent these.\nIncreased confidence in discussing medicines as a risk factor for falls.\nIncreased knowledge in identifying falls risk screening tools currently used in the Australian healthcare setting.\nIncrease confidence in discussing the role of pharmacists in reducing falls risks in older adults.\n\n \nThis workshop is aimed at pharmacy students\, early career and practising pharmacists. \nThis activity has been accredited for 1.5 hours of Group 1 CPD (or 1.5 CPD credit) suitable for inclusion in an individual pharmacist’s CPD plan\, which can be converted to 1.5 hours of Group 2 CPD (or 3.0 CPD credits) upon successful completion of relevant assessment activities. \nRegister here

SUMMARY:Dementia Enabling Environments for Falls Prevention
DESCRIPTION:Persons with dementia are two to three times more likely to fall compared to persons without dementia. Therefore\, older adults with dementia can be considered a high-risk group for falls. DESCRIPTION:With Western Australia’s ageing population\, family carers are increasingly becoming essential support in falls prevention and care delivery for older adults living in the community. Carers’ concerns for older adults’ falls risk\, can have a severe impact on the psychological well-being and ability of family carers to prevent falls. Falls prevention programs that include family carers contribute to significant improvement in falls risk awareness and knowledge in carers and their care recipients. However\, maladaptive caring strategies adopted by carers could lead to dependence and increasing risk of falling among older adults at home. \nJoin Injury Matters for the ‘Understanding family carers fall concern and their management of older adults fall risks at home webinar’. A presentation will be provided by Marcus Ang\, Edith Cowan University who will discuss issues that contribute to concerns of family carers regarding risk of falling and the strategies they use to prevent older adults from falling at home.
